We appreciate your interest in establishing a relationship with AVRECO! Before we can bind coverage, the above linked documents must be completed and returned to our Chicago office to the attention of Nicole Lindberg (please note that you got our Broker Agreement and Agency Profile off our website). In addition to the completed Broker Agreement and Agency Profile, we also need the following:

• Proof of current professional liability (E & O) coverage
• Current resident business entity license and agency principal’s resident producer license
• W-9 Form

For your convenience, we have made the necessary documents into Word forms. Please scan and email the completed documents to the following email address: nicole_lindberg@rpsins.com. (Again, please note that you got our Broker Agreement and Agency Profile off our website). Once we have received all completed documents, you will receive a scanned countersigned copy via email.
