Before we can bind coverage, the above linked documents must be completed and returned to our Chicago office to the attention of Karen Zyck (please note that you got our Broker Agreement and Agency Profile off our website). In addition to the completed Broker Agreement and Agency Profile, we also need the following:

• Proof of current professional liability (E & O) coverage
• Current resident business entity license and agency principal’s resident producer license
• W-9 Form
